Registration Information
What to Expect When You Register at South Bay Hospital
The hospital registration area is located at the back of the facility. Please park in the rear parking lot and enter through the Outpatient/Emergency entrance. You will check in with a volunteer at the Information desk that is located on the right hand side as you enter the building. Once the volunteer takes your information, you will be asked to wait for the next available registrar, depending on the number of patients registering, this will normally take about 5-10 minutes.
Once you are called into registration, you will speak with one of our friendly registrars. This person will make copies of your registration information, including identification and insurance card. This person will also take your co-pay and have you sign a consent form. Once you have completed registration, you will be directed to the appropriate area of the hospital. This process typically takes 10-15 minutes if the information you bring with you is complete. To help ensure your registration does not take longer, please review the list below of what to bring and what not to bring to the hospital for registration.
What to Bring to the Hospital
- Personal Identification
Such as a Drivers License or other Photo ID - Insurance Card
- Co-Pay
Please bring a form of payment for your insurance determined co-pay. We accept cash, check or credit card. Other than co-pay, please do not bring other funds with you to registration. - Doctors Orders
Please verify that your orders include a diagnosis and the doctors signature. - List of Medications
Please do not bring your actual medications, only a list of what you are taking. - Advance Directives
If you have Advance Directives, such as a Living Will, please bring the forms to registration. If you would like more information about Advance Directives, please talk to the registrar.
What to Leave at Home
- Valuables
Please do not bring jewelry, funds other than for co-pay, or any other valuables when registering at the hospital. - Medications
Please bring only a list of your medications, not the actual medications.
